How do I get to your office?

What should I wear to therapy?

Please wear clothing which will allow access to the area of the body to be treated, as well as comfortable and supportive shoes.

If your treatment includes pool work, please bring appropriate swim wear (no denim cutoffs allowed) and a towel. When possible, please wear your swim suit to therapy to help increase efficiency.

How can I get started?

A referral is required from your physician with a recent date and diagnosis. Insurance information is required to ensure proper authorization and coverage verification. We make every attempt to schedule your initial appointment within 24 to 48 hours of your request. Proper insurance authorization is necessary.

What should I bring to my first visit?

Arrive to your first appointment with your prescription, insurance card, any special test results and/or reports pertaining to your injury, and appropriate clothing which will allow access to the body part(s) being treated.

How long are visits?

Each visit is approximately one hour in duration. Your first visit may require the completion of some paperwork; and we ask that you arrive 1/2 hour before your scheduled appointment to ensure that adequate time is allowed. Your initial visit will consist of a comprehensive evaluation by a licensed therapist, who will then individualize a treatment plan based on your diagnosis.

What type of insurance do you accept?

We accept most forms of private insurance, Worker’s Compensation Insurance, and Medicare. Often, even if we are not “in network” with your private insurance carrier, we can deliver treatment in a very cost effective manner.

How will my physician follow my progress?

Prior to your physician appointments, we will write a progress note which you will hand carry to your physician. We will also fax a copy to the physician’s office.

What if I need to cancel or reschedule my appointment?

Brazosport Rehabilitation & Wellness, LLC is committed to exceptional customer service in order to enhance your recovery from injury and illness. To accomplish this quality of care, it is extremely important that you attend every one of your scheduled appointments. If you are unable to attend one of your appointments we will do our best to reschedule you. Please see our cancellation policies for more information.

Can I be seen without a prescription?

Yes, as of September 1st of 2019 patients in Texas may be seen/evaluated without a physician’s referral. However, this does not guarantee that your physical therapy visit will be covered by your insurance provider. Please call our office for more information regarding our Referral Disclosure for Physical Therapy Treatment.
